About Coordinated Healthcare
We offer general psychological counselling services and further care management support services if a client chooses. See below for further details. Our Two Types of Care
Psychological Services
We offer psychological counselling by registered psychologists. If you have a valid Mental Health Care Plan, you may be eligible for Medicare rebates. We can see you without a Mental Health Care Plan, but you would need to pay for the services and claim back on your private health insurance if you have it. Our service fees are on a sliding scale and we can Bulk Bill in some circumstances. Our Psychological Services are mainly via Telehealth (phone or video) for your ease of access. Face-to-face counselling can also be arranged. Find out more in the link below.
Care Management
As a separate service, we also offer care management support. A care manager is someone that assists in linking your carers to make sure everyone is up to date with your care needs. These services will be discussed with you so that your needs can be identified. For example, we can assist in arranging a case conference between your doctors and allied health professionals, help you to keep your records all in one place or check with you to see if your health care goals are being met.
